No-Show / Cancellation Policy

A written rule sheet telling patients what happens when they miss an appointment or cancel late, including any fees.

What it usually contains

  • Notice period required to cancel or reschedule without penalty
  • Fee amounts for missed appointments and late cancellations
  • How many missed visits before a patient may be dismissed from the practice
  • Exceptions for emergencies, weather, or hospital admissions
  • Patient signature line acknowledging the policy

Use it to answer how much notice a patient must give, what a no-show costs, whether a fee can be waived, and when repeated missed visits lead to discharge from the practice.

How it is versioned

A document of this type has exactly one current version at a time.
