A pre-printed form listing a practice's common diagnoses and procedures, used to record what was done at a visit for billing.
What it usually contains
- Patient and insurance identifying fields
- Checkbox lists of procedure and service codes with descriptions
- Common diagnosis codes for the specialty
- Provider name, credentials, and tax or national provider numbers
- Charge amounts, date of service, and signature lines
What the assistant uses it for
Answering what belongs on a superbill, how codes are grouped for a given specialty, and what a patient needs on the form to seek out-of-network reimbursement.
How it is versioned
A document of this type has exactly one current version at a time. Upload a revision and it becomes the version that counts — the one before it is kept and dated, but is no longer what your assistant answers from. Nothing is ever deleted, so you can always show what this document said on a given date.
