Disciplinary form templates
A standard form used to document employee misconduct or performance problems and the corrective action taken.
What it usually contains
- Employee name, position, shift, and date of incident
- Description of the policy violation or performance issue
- Level of action: verbal warning, written warning, suspension, termination
- Expected corrective steps and follow-up date
- Signature lines for employee, manager, and witness
What the assistant uses it for
Answer questions about how to record a write-up, what details a warning must include, and the escalation steps between a first warning and termination.
How it is versioned
A document of this type has exactly one current version at a time. Upload a revision and it becomes the version that counts — the one before it is kept and dated, but is no longer what your assistant answers from. Nothing is ever deleted, so you can always show what this document said on a given date.
