A standard form managers use to evaluate a hospitality or restaurant employee's job performance over a set period.

What it usually contains

  • Employee name, position, review period, and reviewer
  • Rating scales for guest service, teamwork, punctuality, and food safety or cleanliness
  • Role-specific duties for servers, cooks, housekeepers, or front desk staff
  • Manager comments, goals for the next period, and training needs
  • Signature lines for employee and manager acknowledgment

What the assistant uses it for

Use it to answer how staff performance is scored, which behaviors and skills are assessed for a given role, and how review meetings, goal setting, and documentation are handled.

How it is versioned

A document of this type has exactly one current version at a time. Upload a revision and it becomes the version that counts — the one before it is kept and dated, but is no longer what your assistant answers from. Nothing is ever deleted, so you can always show what this document said on a given date.
