Change request forms
A form a policyholder submits to change details on an existing insurance policy, such as coverage, address, or named insureds.
What it usually contains
- Policy number and current policyholder details
- The specific change requested and its effective date
- Added or removed vehicles, property, drivers, or dependents
- Coverage limit and deductible adjustments
- Policyholder signature and date, plus agent or processor notes
What the assistant uses it for
Use it to answer what information a client must supply to alter a policy mid-term, which changes require a signature or underwriting review, and when the change takes effect.
How it is versioned
A document of this type has exactly one current version at a time. Upload a revision and it becomes the version that counts — the one before it is kept and dated, but is no longer what your assistant answers from. Nothing is ever deleted, so you can always show what this document said on a given date.
