Intake questionnaires

A form completed by an applicant or agent that gathers the facts an underwriter needs to evaluate and price a risk.

What it usually contains

  • Applicant and business identity details, locations, and contact information
  • Operations description, industry classification codes, and years in business
  • Exposure figures such as payroll, revenue, vehicle or property schedules, and headcount
  • Loss history questions and prior carrier or coverage details
  • Requested coverages, limits, deductibles, and effective dates
  • Eligibility knockout questions and signature or fraud-warning language

What the assistant uses it for

Use it to see what information must be collected before a risk can be quoted, and which answers trigger referral, decline, or requests for supporting documents.

How it is versioned

A document of this type has exactly one current version at a time. Upload a revision and it becomes the version that counts — the one before it is kept and dated, but is no longer what your assistant answers from. Nothing is ever deleted, so you can always show what this document said on a given date.
